1.) A Referral from your assigned PCM on post; OR
NOTE: Please provide your PCM a reason for your need in seeking services from a Non-Network/Out-of-Network Provider.
Reasons for your need may consist of:
- No other In-Network Provider in the area providing the services that you need;
- For your Continuity of Care that you remain with a previous provider that you've already established care with.
2.) Point-of-Service Benefits, if you choose services without a referral from your assigned PCM.
NOTE: Please verify with your Tricare benefits. This typically consists of you needing to meet a $300 deductible and then once this is met, you will will be responsible for 50% cost share each session (i.e., you'll pay 50% of what the provider's private pay rate is for the service you receive).
